Within the Health Department, the Ponca Tribe of Nebraska’s Special Diabetes Program for Indians Grant, which serves all Tribal Nations, offers the Diabetes Program. The Diabetes Program follows the I.H.S. Standards of Care for the Treatment of People Living with Diabetes.
To qualify for coverage and services, Tribal members must provide the following:
- Proof of diabetes diagnosis
- Copy of Tribal enrollment
