Dr. Jacqueline J. Bae, the CEO of Ponca Health Services, has over 28 years of experience in the healthcare industry, serving as a medical doctor, professor, and health administrator. She has 15 years of experience with Tribe/IHS/UIHS, working in various roles including CEO, COO, Health Operations Officer, Health Director, Tribal Health Officer, and Director of QA/Community Health. Dr. Bae’s expertise includes AAAHC, compliance with NCQA, the Joint Venture Construction Program, 105L Lease, grants, QAQI, training/education, risk management, and overall operations. Dr. Bae is married and has a son. She enjoys visiting South Korea, her homeland, and spending time with her family, reading, and watching movies.

PAUL ROZEBOOM, MD 
Family Physician & Pediatrician 
Fred LeRoy Health & Wellness Center | Omaha, NE 

Dr. Rozeboom has been a pediatrician for 33 years. He was born and raised in Winterset, Iowa. He received his B.A. in Biology from Drake University in Des Moines, went to medical school at The University of Iowa in Iowa City, and completed his Pediatric residency at Blank Children’s Hospital (Iowa Methodist Medical Center) in Des Moines. Prior to joining Ponca Health Services 2 years ago, he practiced as a general pediatrician in Wisconsin, Minnesota, & Iowa. He also worked as a locum tenens pediatrician in 2016 at Crow/Northern Cheyenne Hospital on the Crow Reservation in Montana; this experience led to his continued desire to work with the Native American population and his ultimate decision to join Ponca Health Services.

Dr. Rozeboom is certified by the American Board of Pediatrics and is a fellow of the American Academy of Pediatrics.

Rachel Bogacz, PCMSW, PLMHP
Director of Integrated Care

Rachael Bogacz, the Director of Integrated Care, is a member of both the Ponca and Omaha Tribes. She resides in Omaha and works from the Fred Leroy Health and Wellness Center. Rachael has worked with the tribe for 3 years. Prior to her current position she served the tribe as a Behavioral Health Coordinator and Grant Manager. She attended the University of Nebraska in Omaha for her undergraduate and graduate degrees. She received her Bachelor of Science in psychology in 2016 and Masters of social work in 2019. She completed her first practicum as a Triage therapist at Douglas County Community Mental Health Center. She completed her second practicum at Fred Leroy Health and Wellness Center as a therapist under the supervision of Jay Eason, LCSW. In her current position, she manages multiple grants, provides direct client services, works on implementing integrated care in the clinics, and supervises case management and youth group staff. She also works closely with the Chief of Behavioral Health in continuing to improve and grow the department. She states that she enjoys her job and she is grateful that she is able to give back and provide services to tribal members.

KATIE LADEHOFF, APRN, FNP-C 
Family Nurse Practitioner

Katie Ladehoff is a Family Nurse Practitioner with over 12 years of healthcare experience. She is board certified through the American Academy of Nurse Practitioners. She has provided care at the Fred Leroy Health and Wellness Center for almost 3 years. Her focus is on truly getting to know her patients and collaborating to make their health concerns and goals a priority. She enjoys women and children’s health as well as managing chronic conditions such as diabetes, depression and anxiety. Her professional expertise includes a variety of skills and knowledge. Katie has worked as a registered nurse (RN) in several areas including labor and delivery, inpatient and outpatient psychiatry, peri- and post-operative services, and family practice. She has worked as an APRN as a family nurse practitioner for a private clinic that specialized in hormone optimization, weight loss and thyroid management, and has worked for a correctional facility providing healthcare to inmates. She lives outside of Cedar Creek with her husband Colby; daughter, Kinlee; 2 cats, Birdie and Don; and Small Munsterlander dog, Simon. In her spare time, she enjoys rescuing and fostering animals, gardening, canning, cooking and being outdoors.

REBECCA LECHNER,  DNP, APRN, FNP-C 
Family Nurse Practitioner

Rebecca Lechner is a Family Nurse Practitioner and a member of the Ponca Tribe of Nebraska. Rebecca is from small town Neligh, Nebraska, the daughter of Ray & Deb Branstiter of Neligh, and the granddaughter of Theodore (Red) & the late LaRita Branstiter of Niobrara. Rebecca graduated from Nebraska Methodist College in Omaha, NE with her Doctorate in Nursing Practice. Rebecca served many years as a Registered Nurse in the acute care and emergency care settings.  

 Rebecca says, “As a Family Nurse Practitioner, I have been trained to care for the whole family, from infancy to the elderly. I can assess, diagnose and treat the majority of conditions which present to the clinic.” She understands, though, that some more complex conditions may require further evaluation by a specialist or physician and has built great rapport with many providers around the Lincoln community already. “I am passionate about education and teaching my patients about maintaining a balance for optimal health.” 

JACKIE SCHANTZ

Jackie Schantz is a Family Nurse Practitioner that graduated from the University of Nebraska Medical Center. She also completed her bachelor’s in nursing science through UNMC. She enjoys taking care of and getting to know families and building relationships with her patients. Her desire for patients is to be proactive in their health to prevent and limit chronic disease allowing for maximum quality of life. Her practicum experience includes primary care, critical access medicine, pediatrics, and women’s health.  Prior to being a nurse practitioner, she was a registered nurse for 10 years working in critical care, ER, med-surg, pre-operative and recovery, family practice, and orthopedics. In her free time, she enjoys farming with her family, running, singing, and being active in her church.

HEATHER TALBOTT, MSN, APRN

Heather Talbott, MSN, APRN is a Family Nurse Practitioner at Ponca Health Services at the Lincoln location at 1600 Windhoek Drive. Heather joined the practice in April of 2022 and is now accepting patients of all ages. Heather is a native of Summerfield, Kansas but has called Lincoln home for over 20 years.  She is a graduate of Nebraska Wesleyan University where she earned her Master’s in Nursing Degree in Leadership and Management in 2014. She furthered her education at Clarkson College where she received her Post-Master’s Degree as a Family Nurse Practitioner in 2017. She began her medical career in Trauma and Critical Care Medicine and most recently has been specializing in the management of adult and geriatric patients. Heather is a member of the American Academy of Nurse Practitioners. She has two children and stays busy with their sports activities. When she does have free time, she enjoys live music and spending time with friends and family.

JENNIFER BROWN, MA, LMHP, LPC, LADC
Master’s Level Therapist, Licensed Mental Health Practitioner, Licensed Professional Counselor, Licensed Alcohol and Drug Counselor

Receiving her Master’s Degree in Mental Health Counseling from Doane College in 2012, Jennifer holds experience in treating adults with both mental health and substance use issues as well. She has worked as a Licensed Alcohol and Drug Counselor since 2007. She enjoys working with couples and individuals. Jennifer’s areas of specialty are anxiety, depression, women’s issues, couples counseling, cultural identity, historical/intergenerational trauma, and substance abuse/dependence. Jennifer is an EMDR-trained clinician (Levels I and II), a Brainspotting Trained Clinician (Levels I and II), and a Somatic Archaeology© practitioner. She is a member of the Southeast Nebraska EMDRIA Regional Network.

Jennifer believes in a holistic and integrative approach to health and wellness, including; spiritual, emotional, mental, and physical health. Jennifer became a therapist to assist others in their personal journey towards health and wellness. She is a member of the Oglala Oyate of Pine Ridge, SD. She is a mother with two children, a grandson, and one fur baby, and she is a passionate nature enthusiast.

JASMINE STEPHENS, PLMHP, PLADC | LICENSED CHILD/ADOLESCENT THERAPIST

My name is Jasmine Stephens, and I am a provisionally licensed mental health practitioner (PLMHP) and a provisionally licensed alcohol and drug counselor (PLADC). I was hired by the Ponca Tribe of Nebraska in January of 2023. In sessions, I utilize a variety of techniques including but not limited to Cognitive Behavioral Therapy (CBT), motivational interviewing, Dialectical Behavioral Therapy (DBT), mindfulness-based therapy, and person-centered therapy.

Will Freiberg, an enrolled member of the Ponca Tribe of Nebraska, is a clinical pharmacist for Ponca Health Service (PHS). As a clinical pharmacist, Freiberg assists PHS providers in providing high-quality healthcare to our patients. Freiberg previously served as a clinical pharmacist for the Catawba Indian Nation in Rock Hill, SC from 2015 to 2020 and as a clinical pharmacist and pharmacy supervisor at Oyate Health Center (formerly known as Sioux San Hospital) in Rapid City, SD from 2010 to 2015. Freiberg received a Doctor of Pharmacy degree from South Dakota State University in 2009 and a Masters in Healthcare Administration from Oklahoma State University in 2019. He is currently dual-board certified by the Board of Pharmacy Specialty (BPS) in Pharmacotherapy (BCPS) and Ambulatory Care (BCACP).

Dr. Cameron Aitken, DDS
Dr. Aitken is a 2021 graduate of the University of Nebraska Medical Center College of Dentistry.  He currently serves as the Chief Dental Officer for the Ponca Tribe of Nebraska, a position he took in April 2024.  Prior to the CDO position, Dr Aitken served as the Staff Dentist in the Norfolk PTN Clinic from June 2021-November 2022 and in the Lincoln PTN Dental Clinic from November 2022-April 2024.  Dr Aitken looks forward to making and/or maintaining a smile patients feel comfortable in!
