Special Diabetes Program for Indians Grant
Within the Health Department, the Ponca Tribe of Nebraska’s Special Diabetes Program for Indians Grant, which serves all Tribal Nations, offers the Diabetes Program. The Diabetes Program follows the I.H.S. Standards of Care for the Treatment of People Living with Diabetes.
Our program provides comprehensive education, supplies, activities, and nutrition resources to help Tribal members manage diabetes effectively and live healthier lives rooted in cultural wellness.
To Qualify for Coverage & Services
Tribal members must provide the following documentation to qualify for Diabetes Program coverage and services:
Proof of diabetes diagnosis
Copy of Tribal enrollment
