Diabetes Education
The Diabetes Program staff conducts self-management education classes at each office site on a monthly basis.
Nutrition Consultations
A registered dietitian provides nutrition consultations for individuals with diabetes and others who are at risk for developing diabetes. In addition, Medical Nutrition Therapy is provided at the Fred LeRoy Health and Wellness Center and the Ponca Hills Health and Wellness Center.
Nurse Foot Care Clinic
The RN/Case Manager holds bi-weekly nurse foot care clinics at the Fred LeRoy Health & Wellness Center and monthly nurse foot care clinics at the Ponca Hills Health and Wellness Center.
Case Management Services
The Diabetes Program staff provide case management services to patients with diabetes needing more focused one-on-one education and diabetes management.
Eye Exams
The Diabetes Program provides coverage for annual eye exams for individuals with diabetes registered in the Ponca Tribe of Nebraska Diabetes Program.
Direct Care
A medical doctor and a three family nurse practitioners provide direct care to patients with diabetes at the Fred LeRoy Health & Wellness Center and Ponca Hills Health and Wellness Center.
